Register with Disability Resources

Once students have selected the ÂÒÂ×Ç¿¼é and have enrolled, they can register with our office:

  1. Complete the application form. These application forms are also available in PDF format upon request:

  2. Submit supporting documentation
  3. Contact our office to arrange an intake interview. The interview can take place during the summer or once you arrive on campus.

Submit Supporting Documentation

Upon disclosing the existence of a disability, students will be asked to forward supporting documentation to the local access coordinator. Detailed University-wide guidelines for documenting a disability, procedures, and policies are found at: .

Students enrolled in the School of Arts and Sciences and the Hajim School of Engineering should submit documentation of a disability to our office.
